An emphasis on readiness for life after Concord includes individualized transition plans for all high school students. Transition services assist our students in preparing for the future and include: job skills and life skills; resumes, job applications, mock interviews and internships; college planning and applications; understanding disabilities and learning self-advocacy skills.
Transition to Success is a program supported in part by a grant from the Assisi Foundation, with additional support from the Speer Charitable Trust. This program features a comprehensive array of transition services and involves students more deeply and earlier in their high school experience.
Guided by a transition coach, students are immersed in authentic learning opportunities with the goal of increasing their levels of competence and confidence in life skills and job-readiness skills in both school-based lab experiences and real-world settings.
